Case Management
Curry’s Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors, and evaluates the options and services required to meet the seniors’ health and human service needs.
Case Management is characterized by advocacy, communication, and resource management to help our seniors live in their own community. Services are provided to seniors in their own homes, apartments, single-room occupancy units, or homeless shelters.
The Program is open to all seniors over the age of sixty or individuals with disabilities who live at or below the poverty level and are residents of San Francisco. There is no cost or fee for this program. Our Behavioral Health Department specializes in serving individuals with Medi-Cal, but we will work with every prospective client to provide services and coordinate care regardless of their insurance status.
At Curry’s 315 Turk Street Office, Case Management and Behavioral Health are open Monday through Friday, 9am to 4pm, and closed for lunch from 12pm to 1pm. Appointments are not necessary to speak with the Officer of the Day.
